What You Will Do To process all forms of requests for protected health information (PHI) by abstracting information from multiple Sharp HealthCare (SHC) systems.
Request types include subpoenas, audits, sensitive information, etc.
Ensures they are valid, accurate, appropriate, and completed within established timeframes, in accordance to state and federal laws, while meeting the goals and mission of SHC.
Functions include record retrieval, faxing, mailing, e-mailing, e-delivering, copying, filing, data entry, customer service, essential reception skills, all while protecting patients' privacy.
Required Qualifications 1 Year experience in a Health Information Management department.
Experience with speaking to customers on the phone and in person.
Preferred Qualifications ROI experience.
Data entry experience.
Certified in Healthcare Privacy and Security (CHPS) - The American Health Information Management Association (AHIMA) -PREFERRED Registered Health Information Technician (RHIT) - The American Health Information Management Association (AHIMA) -PREFERRED Other Qualification Requirements Associate’s degree (or higher) in Health Information; or two years relevant experience may substitute for degree - Required Essential Functions Department Operations Receives and/or assists customers in proper completion of requests for release of medical record information according to established policy and procedure requirements.
Receives, logs in, processes and tracks release of information requests in accordance with Confidentiality and Release of Information policies and procedures.
Provides appropriate feedback to requestors in a timely manner.
Maintains electronic correspondence log system in accordance with established procedures.
Answers incoming calls in a timely manner and responds to caller’s needs.
Return calls left on voicemail within 24 hours.
Encourage self-service through promotion of the patient portal and send sign up invitations to patients.
Provides courteous and professional customer service support for the department by assisting physicians, patients, and any other requestors with ROI.
Answer the telephone within three rings and help with customer walk-ins.
Promotes and fosters positive customer service experiences.
Assures that all ROI requests are prioritized and processed accurately in accordance to SHC Policies, Procedures and Guidelines, HIPAA, CMIA, LPS, and other laws pertaining to the ROI.
Always ensure that necessary documentation has been obtained in order to meet the legal standards for releasing records, both sensitive and non-sensitive.
Documents all requests and notes in the electronic tracking system in accordance with established procedures.
Process all customer requests within 15 days of receipt, unless otherwise stated.
Process all subpoenas in accordance to the directions on the subpoena.
Meets turnaround times.
Ensure that all requests are reviewed within 24 hours of being assigned.
Provide copies and support Legal and Risk Management when needed.
Consults with direct leadership, quality, Risk Management, and Legal as needed to release records on vague or difficult authorizations.
Ensures records containing sensitive information are processed appropriately.
Retrieve documents from all appropriate systems for ROI.
Fulfill the role of SHC custodian of records to include attending depositions, hearings, and/or trials.
Responds to public inquiries about release of medical information, the status of the request or general questions regarding Release of Information.
Practices closed loop communication.
Properly creates, responds to, and completes electronic tasks within the electronic record.
Utilizes and monitors shared inboxes.
Communicates with vendor as needed.
Verify and confirm that ROI requests match the patients' information at SHC.
Validate patient and/or patient representative's signature on the ROI request.
Research and gather all relevant information and dates of service authorized on the release of records.
Order any necessary paper records and/or documents to fulfill the request.
Enter requests into the ROI tracking system and appropriately assign.
Prioritize requests by due date and notify other Med Legal Specialists as needed.
Scan requests into the electronic health record (EHR) to maintain proper accounting of disclosures.
Process weekly check deposits.
Request patient medical records utilizing the established electronic chart locator system.
Screen each request for information to determine urgency and assures that most urgent requests are completed first using established priority systems.
This includes receiving, sorting and organizing work in accordance with time category (stat, urgent, routine) and simple or complicated release types (including medical, subpoena, disability), and utilizing standardized naming conventions related to releases.
Promotes the patient portal and offers assistance with enrollments and use.
Processes requests for proxy access to patient portal and web-based bill payment.
Collects, reviews for accuracy and submits accurate individual, departmental productivity reports, and quality statistics by established due date.
Quality Assures that all work completed is in accordance with the quality of Sharp Policies and Procedures as well as that of a High Reliability Organization (HRO).
Review all ROI requests for appropriateness, validity, and HIPAA compliance before disclosure of any PHI.
Follows established guidelines for ROI in accordance with HIPAA Privacy, Security Rules and applicable laws.
Perform quality control of the paper and electronic health record, checking for both data integrity and image quality.
Demonstrates job ownership.
Ensures low supply levels are reported to department leadership.
Completes any reporting or quality related tasks assigned by leadership.
Observe, evaluate, and communicate with management.
Actively participates in performance improvement opportunities.
Timely reporting of and assistance with duplicate registrations.
Report any errors in indexing and documentation.
Scanner equipment maintenance: cleans, maintains, and calibrates scanning equipment.
Monitors assigned tasks in electronic health record (EHR) placing them "In Progress" within two hours or receipt and accurately documenting all updates in the details section of the tasks.
Properly complete all assigned tasks.
Training Demonstrate knowledge and assist with introducing employees/students and others, as assigned, to the ROI processes.
Train and orient new staff members to EHR, ROI tracking system and department operations.
Submit timely feedback to Lead or Supervisor.
Serve as a mentor for assigned trainees.
Meetings Attends and is active during all assigned meetings.
