Vivo HealthStaff is looking for a *licensed LVN* for a healthcare organization in *San Marcos, California.
*
The Clinic LVN is responsible for coordinating and providing direct and indirect nursing care to participants at the center as well as at the participant's home, working in close coordination with the PACE Physician, Registered Nurse, clinic staff, and other interdisciplinary team (IDT) members to manage ongoing care, complex clinical situations, and to support the operation of the GMWP primary care clinic.
*Essential Functions:*
* Works under the supervision of the Clinic Director/Registered Nurse and maintains vocational nursing professional standards of care.
* Performs home visits to administer prescribed medications and treatments by nursing standards to supplement clinic services.
* Observes, record, and report participant's condition and reaction to medication and treatments to physicians.
* Documents all interactions related to clinic visits in the electronic medical record.
* Instruct participants and family members regarding medications and treatment instructions in the home by CNAs, PCAs, and Caregivers.
* Keeps Home Care Supervisor and Interdisciplinary team informed on home care issues of participants.
* Attends morning meetings at the center, family meetings, and any other meetings as assigned.
* Follow GMWP policies and procedures and participate in any required Quality Improvement activities, and staff training.
* Other duties as assigned.
* Requires use of personal vehicle.
* Proof of auto insurance listed as a driver.
* Active valid driver's license.
*DUTIES/RESPONSIBILITIES:*
* Processes, communicates, and implements all medication orders.
* Administers and distributes all medications as prescribed by medical staff.
* Provide nursing care and treatments for participants at the Day Health Center.
* Maintain clinic supplies.
* Participates actively in the interdisciplinary team by offering clinical expertise in team planning, problem-solving, and processes.
* Teaches, supervises, and counsels the participant, geriatric aides, and family members regarding nursing care needs and other related problems of the participant.
* Other related duties as assigned.
*QUALIFICATIONS:*
* Graduate of an accredited LVN program.
* Possess a current California LVN license.
* Interest, demonstrated ability, and one year of experience working with the elderly or frail population required.
* Ability to work independently and as part of a multidisciplinary team.
* Bilingual Spanish-English preferred.
