Program Type: Community-based To Make Referrals: The county of San Diego has determined that all persons referred to the Telecare AOT program shall initially be screened and evaluated by the existing In Home Outreach Team (IHOT) program.
Referrals to AOT can only be made through Telecare IHOT.
AOT examinations will be conducted by a licensed clinician from the IHOT program.
# of Members: 70 members Who We Serve: Adults with serious mental illness who are experiencing repeated hospitalizations or incarcerations but are not engaging in treatment.
Only individuals who meet the nine criteria as established under Laura’s Law are eligible for referral to AOT: At least 18 years of age.
Have a serious mental illness.
Determined unlikely to be safe in the community without supervision.
History of non-compliance with treatment that has either: Been a factor in their hospitalization and/ or incarceration at least twice in the last 36 months, or resulted in one or more acts, attempts or threats of serious and violent behavior toward self or another within the last 48 months (not including current event).
Has opportunity to voluntarily participate in mental health treatment but continues to fail to engage.
Condition is substantially deteriorating.
AOT is the least restrictive placement necessary to ensure recovery and stability.
Needs AOT to prevent a relapse or deterioration that would likely result in grave disability or serious harm to self or others.
Be likely to benefit from AOT.
About the Program: The San Diego Assisted Outpatient Treatment (AOT) program provides outpatient services to adults with serious mental illness who have been court-ordered to receive services under Laura’s Law.
Program members may also have a co-occurring substance use diagnosis.
Treatment is provided by a multidisciplinary team, and all services are strengths-based and anchored in recovery principles.
Individuals identified through assessment that do not voluntarily agree to services are required to go before a judge to determine the need for treatment.
