Med-Legal Company is seeking a full time Nurse Case Manager in San Diego to help meet the clinical, rehab and medical needs of our clients in the San Diego & Southern California area.
THIS IN AN IN-OFFICE POSITION - REMOTE WORK IS NOT AVAILABLE.
YOU MUST BE IN THE OFFICE MONDAY-FRIDAY EXCEPT WHEN AT PATIENT APPOINTMENTS.
THIS JOB REQUIRES DRIVING TO AND FROM APPOINTMENTS IN SAN DIEGO.
Responsibilities
· Assessing, planning, implementing, coordinating & evaluating the care needs of disabled adults/children with catastrophic injuries/illnesses.
· Demonstrates knowledge of treatment options, clinical diagnoses, and ability to accurately describe and interpret significance of findings.
· Coordinating resources and treatment options for our clients.
· Proactively identifying problems, issues, options and opportunities regarding the well being of our clients.
· Create reports and provide justification letter to the courts for special need trust clients.
· Advocate for the welfare of our clients’ and serve as liaison between client and their families and health care providers.
· Facilitates Client/Family access to community, state and federal resources to provide quality and cost effective services.
· Establishes relationships with clients, families, providers and resources within the community.
· Preparing timely and accurate monthly/quarterly reports.
Qualifications
- Must live in or be moving to the San Diego area
· Current California RN Nursing license required.
- Associate's or Bachelor's Degree in Nursing required.
· At least two (2) years of acute nursing experience required.
o Experience in rehabilitation or case management preferred.
· Bilingual (English/Spanish) or fluency in any language along with English preferred.
· Able to make the client and their care their primary focus.
· Must be a self starter.
· Able to work both independently and as part of an interdisciplinary team.
· Computer literacy (including internet research & Microsoft office) required.
· Excellent communication skills, both verbal and written required.
· Assertive and calm in challenging situations.
· Projects a positive and professional image and a dedication to teamwork and the well-being of the client, especially during difficult periods.
