Job Title/Position: Resource Nurse
Reports To: Team Leader
JOB DESCRIPTION SUMMARY
Our Resource Nurse is primarily responsible for the evaluation and admission of potential hospice patients according to Medicare guidelines.
The Resource Nurse mainly focuses on daily PRN visits and admitting patients into service.
All members of the Cabrillo Hospice clinical teams ensure appropriate communication and plan of care coordination to all internal/external customers.

Responsibilities:
The Resource Nurse evaluates and admits qualified patients to hospice service.
Conducts PRN visit when needed.
Communicates all changes to RNCM or IDT and primary MD.
Serves as a resource to the Cabrillo Team and community partners.
Performs job duties in clinical areas as stated in clinical competency checklist, policies/procedures, standards of care, and critical competencies based on Scope of Practice.
Carries out the Cabrillo belief that relationships go far beyond the minimum care required.
Maintains timely patient documentation using “point of care” electronic charting.
Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements.
Provides health care instruction to patient and family members.
The Resource Nurse orients, instructs, and trains assigned personnel.
Performs admission assessments.
Primary Service Locations: San Diego County
Schedule: Monday - Friday 8-hour shifts
Pay: $48.
00-$60.

Qualifications
1.
Current CA RN license.
2.
At least 1 year of experience as a professional nurse within the last 3 years.
3.
Current CA driver’s license with proof of insurance.
4.
Computer skills [MS Word and Electronic Medical Record (EMR) system]
5.
Excellent observational, written, and verbal communication skills.
Understands hospice philosophy and issues of death/dying.
6.
CPR certification required.
