Community Health Workers (CHW) are frontline public health workers who will engage with and connect individuals, families, care and community organizations to better understand the services needed.
Community Health Workers build individual and community capacity by increasing individuals' knowledge and self-sufficiency through a range of activities such as outreach, community education, social support and advocacy.
Community Health Workers (CHWs) are frontline public health workers who are trusted members of the community they serve.
CHWs serve as liaisons between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
Incumbents may be assigned to varied programs and work locations within the Health and Human Services Agency (HHSA), reporting to the Department of Homeless Solutions and Equitable Communities (HSEC) and Public Health Services.
NOTE: Incumbents may occasionally be required to work during irregular business hours such as evenings.

Job Offer and Background Process begins week of March 11, 2024.
Pre-employment Medical Screening: Approximately 3 days following successful background investigation results.
Anticipated Start Date: Start date will be based upon successful completion of medical screening or other mutually agreed upon date.
If you are not selected for this interview process, you may have the opportunity to compete for future vacancies.
Minimum Qualifications: Qualifying education/experience: High school degree or GED AND At least one (1) year of full-time experience or one (1) year of full-time equivalent volunteer experience providing social services and/or health education and outreach to the community.

Evaluation: The names of qualified applicants will be placed on a six (6) month employment list based on scores received during the evaluation of information contained in their employment and supplemental application.
