PROGRAM SUMMARY
Action Central (Assertive Community Treatment) is a dual-track treatment program for individuals with severe mental illness and/or a chronic substance use disorder.
The program provides therapeutic interventions, peer support, self-management skills, medication management and monitoring, supportive housing services, comprehensive outpatient and community services, and more.
JOB SUMMARY
The Licensed Vocational Nurse (LVN) dispenses medication, takes and records vital signs, reports adverse reactions to medication or treatment to medical personnel.
Assesses symptomology and provides crisis intervention.
RESPONSIBILITIES

Serves as an active member of treatment team, providing medical care and support to clients.
Monitor clients in the Patient Assistance Program; properly chart and complete documentation in the electronic health record.
Coordinate medication orders and refills with pharmacies.
Respond to client calls regarding medication issues.
Provide client education regarding medications and their side effects.
Dispense and administer prescribed medications.
Collaborate with the Registered Nurse regarding client well-being and medication compliance.
Respond appropriately to medical emergencies.
Take and record vital signs.
Ability to create written documentation that is legible, clear and concise
Demonstrate knowledge of cultural and socio-economic factors that inform service provision to persons with psychiatric or dual diagnosis disorders; demonstrated ability to engage culturally diverse populations
Demonstrate ability to provide counseling and clinical services.
Ability to communicate verbally and interpret, explain and advise about medical information to clients, staff, family members and community providers
Comply with all MHS and program rules, policies and procedures including HIPAA standards.

QUALIFICATIONS
Completion of a licensed Vocational Nursing Program from an accredited college or university
Current licensure as a Vocational Nurse in California
Minimum of three years of experience treating psychiatric and dually diagnosed population
Bilingual English/Spanish if the position serves Spanish speaking population highly desirable
Ability to work independently and effectively collaborate as part of a team
Ability to problem solve and react appropriately to emergency situations
Proficient with Microsoft Office Suite and EHR software
Valid California driver’s license and current auto insurance coverage
