*Your Role in our Company Will Be*
*The LVN supports the Director of Nursing, Case Management and the nursing staff in the delivery of health care by performing patient care assessments, and a variety of medically related tasks.
The LVN is a licensed nurse who works in an Home Care Setting to provide nursing services.

* Our patients need care 7 days a week, although you have flexibility in your scheduling, we do ask that you are able to work at least two-three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer)

Requirements
*RESPONSIBILITIES:*
* Providing nurse visits to home health patients.

* Care for ill, injured, or convalescing patients by Implementing plan of care initiated by RN
* Provide basic patient care and treatment.
Document consistently with the plan of care.
* Communication with patients to schedule visits.
* Educating the patient how to take care during non-visit days
* You'll demonstrate knowledge of regulatory requirements.
