Under general supervision, the licensed clinical social worker provides all aspects of psychosocial services including diagnostic evaluations, assessments, treatment planning, care coordination, case management, and brief individual and group therapy in the Outpatient Psychiatry Services- Hillcrest.
Interviews patients and facilitates discussion around diagnosis and treatment planning.
Provides case coordination, client advocacy, and brokerage to obtain services as needed.
Facilitate integration of patient’s care with primary care providers.
Provide brief problem-focused individual therapy, facilitate therapy groups to be delivered to patients with ongoing mental health needs and those who require bridge-care services while waiting for access to specialty services.
Serve as liaison with other Division providers.
Participates in multi-disciplinary treatment team.
MINIMUM QUALIFICATIONS A Master's Degree in Social Work.
LCSW issued by the State of California.
A minimum of three (3) years post Master's Degree (MSW) experience.
Knowledge and abilities essential to the successful performance of the duties assigned to the position.
PREFERRED QUALIFICATIONS Bi-lingual in Spanish.
SPECIAL CONDITIONS Must be able to work various hours and locations based on business needs.
Employment is subject to a criminal background check and pre-employment physical.
Pay Transparency Act Annual Full Pay Range: $92,841 - $122,315 (will be prorated if the appointment percentage is less than 100%) Hourly Equivalent: $44.
