Hospice Registered Nurses Salus Hospice Salus Hospice is currently looking for Hospice Case Managers for our expanding hospice team.
This position would cover Central/North San Diego County areas including La Jolla, Oceanside, and the surrounding areas.

Hospice Registered Nurse Case Manager Responsibilities: * Perform RN duties on hospice team.
* Serve as a RN Case Manager for the hospice Interdisciplinary Team * Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action * Establishes plan of care while collaborating and communicating with the Interdisciplinary Team including physician when appropriate * Coordinates all hospice services provided to patients/families in all settings to ensure quality of care Ideal Hospice RN Case Manager Candidate Background: * Registered Nurse with valid CA state license * Minimum one year of hospice or home health experience * Strong background in medical/surgical, ICU/CCU, or ER is helpful * Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patients place of residence * Must have excellent interpersonal skills and a passion for hospice * Understanding of hospice philosophy and needs of the terminally ill * Self-directed with ability to work with little supervision * Current Physical and TB test results RN Case Manager Benefits and Perks: *Competitive Compensation Package *Medical/Dental/Vision - Full Time Benefit *401k *Sick Leave *Vacation pay - Full Time Benefit *Holiday pay - Full Time Benefit *Verizon Wireless Discount *Corporate Discount Plan on Theme Parks, Attractions, etc Please apply today!
