Radiant Hospice seeks a compassionate Hospice RN experienced with providing direct in-home care to patients approaching end of life.
At Radiant Hospice, our focus is not on when life ends, but on providing the best quality of life.
We provide personalized care to our patients, ensuring comfort and peace of mind to the patient and their family.
The Hospice RN completes the Initial Assessment to evaluate a patient's physical condition and develops a plan of care that address any of the patient's medical needs or needs for social and spiritual services.
The Hospice RN collaborates with the IDG team to ensure continuity of care, and possesses knowledge of the Medicare hospice benefit and provides explanation of benefits to hospice patients and their families.
Apart from providing comfort to patients, The Hospice RN is adept at counseling and guiding patient's families through the hospice admission process.
Minimum Qualifications:
* Graduate of an accredited nursing program.
* Current California Registered Nurse license.
* At least one (1) year of hospice or palliative care experience
* Minimum of two (2) years of experience as a professional nurse within the last five (5) years.
* Expertise in pain management, symptom control, crisis intervention, and family support.
* Excellent demonstrated critical thinking skills and superior clinical judgment.
* Excellent computer skills.
* Excellent customer service skills.
* Reliable cell phone with local number and coverage.
* Valid driver’s license, proof of acceptable automobile insurance coverage, and reliable personal transportation.
* Ability to work independently and manage time effectively.
Ability to make accurate, and at times, quick judgments.
Ability to supervise others appropriately.
Ability to respond appropriately to crisis outside of a hospital setting.
* Acceptance of and adaptability to different social, racial, cultural, and religious modes.
