Role Overview AND Purpose
The Pharmacy Services Coordinator works to support the mission and vision of Neighborhood Healthcare by providing excellent patient-centered care and services within a dynamic refill team environment.
This role will assist the pharmacist by assessing patient needs to determine appropriate services and care.
Additionally, this role will receive calls from patients and pharmacies regarding pharmacy-related matters.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Refill medications on behalf of a provider according to the Neighborhood’s Pharmacy Technician Refill Protocol
Orders labs needed to monitor medications on Neighborhood’s pharmacy technician refill protocol, as needed
Orders outstanding routine labs required for health maintenance
Maintains accurate and complete records in the electronic health records of all patient encounters
Communicates via the EMR with other NHC staff related to patient care in accordance with the Neighborhood’s Pharmacy Technician Refill Protocol
Attends and participates in required meetings and trainings
Performs all job functions with the Neighborhood’s mission, vision, values, and strategic plan in mind
Maintains a high level of professionalism with all patients, including upset or difficult patients
Refill Center:
Learns and navigates Neighborhood’s electronic medical record (EMR) to investigate refill requests initiated by patients and pharmacies
Reviews current medication lists to identify duplicate or missing medications on patient medication lists
Contacts patients and pharmacies to clarify medication discrepancies prior to refilling a medication
Works with Clinical Pharmacist to secure authorization prior to adjusting medications
Pharmacy-Related Calls:
Creates positive patent experience by answering questions and offers information related to patient-focused services
Creates telephone encounters (TE), including appropriate fields and assigns to the correct care team inbox
Educates patients on methods to contact providers for medication requests and department process for address pharmacy requests, including response and completion timelines
Works in collaboration with refill center and prior authorization center staff to address patient and pharmacy requests
Determines when complex patient inquiries or complaints should be transferred to a supervisor
Quality Management:
Contributes to the success of the organization by participating in quality improvement activities
Cooperates in accomplishing department goals and objectives
Customer Relations:
Responds promptly and with caring actions to patients and employees
Maintains professional working relationship with all levels of staff, patients, and the public
EDUCATION/EXPERIENCE
High school diploma/GED required
Valid Medical Assistant Certificate, or valid California Board of Pharmacy license required
One year experience working within a hospital or outpatient clinic setting required
Current BLS CPR certification required
Experience within pharmacy refill center or prior authorizations preferred
NABP National Pharmacy Technician Certification preferred
Bilingual (English/Spanish) required by site
Neighborhood Healthcare requires employees to be fully vaccinated (including a booster dose) against COVID-19 and demonstrate proof of vaccination/booster upon hire or receive approval for an exemption based on a medical reason, disability or religious belief.
Proof of booster dose will be contingent upon booster eligibility.
Pay Range: $21.
00 - $25.
00 per hour, depending on experience (DOE)
