Under the supervision of the RN, the licensed vocational nurse is primarily responsible for assessing, implementing, and coordinating the individualized plan of care in accordance with physician's orders for the elderly, chronically ill, and those with rehabilitative or palliative needs.
Implements the plan of care in collaboration with residents, families, and other health team members to positively impact clinical outcomes.
Performs designated tasks/duties including: providing information to residents/families and other health team members; participation in Interdisciplinary Team (IDT) rounds; re-assesses continued care plan within scope of practice; documents re-assessment and implementation of plan; consults with RN for modification of plan as needed.
Responsible for monitoring the CNAs to ensure that care is provided with dignity, and to ensure a safe and comfortable environment.
Understands and adheres to the California Licensed Vocational Scope of Practice and The American Nurses Association (ANA) Code of Ethics for Nurses.
Speak and read English at a level that is sufficient to satisfactorily perform the essential functions of the position.
Knowledge of standard office equipment (i.
e.
, calculator, fax, photocopier) and personal computer and computer software skills (i.
e.
, MS Windows, Excel, Access, Word, Powerpoint, internet, e-mail).
Windows computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job.
